Getting Started

Step 1 

Contact Rise Therapy - Call or email to discuss your concerns and determine whether an occupational therapy evaluation may be beneficial for your child. You may also ask your child's primary care provider to send a referral directly to Rise Therapy. 

Step 2

Complete Registration Forms - Complete our online intake forms and register with Rise Therapy's secure medical record system. 

Step 3

Schedule an evaluation - Once we receive your referral and registration information, we will schedule a comprehensive occupational therapy evaluation to assess your child's strengths, challenges, and areas of need. 

Step 4

Insurance Authorization - Rise Therapy will assist with obtaining the necessary physician referral and insurance authorization, if requested. 

Step 5

Develop an Individualized Plan - Following the evaluation, we will review the results with your family and develop an individualized treatment plan tailored to your child's unique needs and goals. 

Step 6

Begin Therapy Services - Therapy sessions take place in your child's natural environments, such as the home, daycare, preschool, or community settings, allowing sills to be practiced where they matter most.
